Community Health Centers of Pinellas is a nonprofit health care organization with 12 locations and a mobile health center. The organization provides affordable primary care services to underserved, low-income, uninsured and underinsured communities.
The Mission of Community Health Centers of Pinellas Inc. is to provide quality health care services to all.
In order to measure the success of its mission, “CHCP recognizes that data is the cornerstone of quality improvement. Several different tools, to include real-time data (i.e., meaningful use, UDS, etc.) are used to assure on-going QI assessments of the appropriateness of service utilization, quality of services delivered, and patient outcomes. CHCP implements targeted QI activities to analyze areas of need, develop interventions and measure the impact of those interventions. The monitoring of quality indicators is designed to reveal trends and performance opportunities in specific areas and facilitate corporation-wide improvement.
“When a QI assessment identifies an area for improvement, CHCP employs multiple methodologies to develop interventions and measure the results of those interventions, such as process improvement through PDSA (plan, do, study, act) cycles for testing. The QI Committee appoints teams to implement PDSA cycles and report the results. The committee decides if the results of a PDSA warrant the implementation of a QIA. The QIA is documented and presented to the QI committee and the board of directors for review and approval and follow up on stated goals and measurements.”
Community Health Centers of Pinellas is one of 36 local nonprofit organizations and businesses that will be honored this year by the Tampa Bay Business Journal for its One Tampa Bay awards.
